In case of loss, theft, or destruction of an original Harvard Design School diploma, replacement diplomas can be obtained by filing a request with the Office of Student Services. To request a replacement send a notarized letter stating the reason for the replacement, copies of insurance, theft, or damage reports, and a check in the amount of $150 made payable to Harvard University. Preparation of the document may take four to six months. When the diploma is ready, it will be sent via insured and certified mail.
