Immunization Requirements and Medical Forms
Massachusetts has strict immunization requirements that you must meet in order to register for classes. Immunizations help protect you from illnesses and contribute to the overall well being of our community. We encourage you to receive any required immunizations before you arrive at Harvard, as many private health plans will cover the cost. If you are unable to obtain these prior to your arrival on campus, you may arrange to get immunizations at various locations in the area, including HUHS. Please note that your health plan may not cover immunizations you receive at HUHS, in which case you will be responsible for the cost of the immunizations.
Please visit the Harvard University Health Services (HUHS) website for additional information and to download the required medical forms that you must complete and submit by June 14, 2019 for the fall term and January 6, 2020 for the spring term.
Massachusetts law requires that all registered students have adequate health insurance coverage. All Harvard University students are automatically enrolled in the Harvard University Student Health Program (HUSHP). Please download and review the HUSHP Letter to Incoming Students for the upcoming academic year issued by Harvard University Health Services.
HUSHP is comprised of two parts: the Student Health Insurance Plan and the Student Health Fee. Visit the HUSHP website for more information and read through the HUSHP Student Letter which provides more information on types of coverage, dates, rates, and waiving the Student Health Insurance Plan.
